Emergency Shelter And Support For Seniors
Seniors over the age of 55 without a “dwelling place” with extenuating medical needs that cannot be accommodated within a communal setting. This program provides temporary/emergency accommodations to seniors.
Cornerstone partners with a variety of motels across the Durham Region to provide temporary accommodation. Since the senior will be unsupervised in the motel setting, referrals to this emergency program must consider the senior’s capacity to exercise reasonable judgment while staying at the motel.
An Independent Living Assessment is completed to determine the individual’s level of dependence on care i.e. total dependence, severe dependance, moderate dependence, light dependance or independence.
After the completion of the independent needs assessment, if it is determined that the client requires PSW support for their ADL’s, or health support Cornerstone will make the referral for support upon entry into the program.
Food services will be provided throughout the duration of stay while in the program (i.e., Meals on Wheels).
The senior will have access to Cornerstone’s internal programs which include mental health support from a psychologist.
Should the senior no longer meet the criteria for this program (e.g., health stabilizes, and the individual is able to access an area hostel, health decreases and the individual cannot live independently in the motel) appropriate arrangements will be made to transition the individual to appropriate programs and resources.
Participants of the program are required to work one on one with a Community and Family Case Manager to reach their goals. Some goals include securing housing, income security, community referrals, medical needs, financial literacy support and any other support the individual may require.
Other Program Criteria:
- Must be financially ineligible to afford alternative emergency accommodations
- Must be unable to access emergency shelter programs due to extenuating medical needs
- Must be able to temporarily stay in a motel without 24/7 assistance of a Personal Support Worker (PSW)
- Eviction must be immediate (i.e., the sheriff is coming on the date they are accessing emergency accommodation, the hospital is discharging them on the date they are accessing emergency accommodation)
- Independent needs assessment and Independent Living assessment must be completed prior to program access.
- Must have legal status in Canada, have applied for legal status in Canada or are refugee claimants and must not be under a removal order to leave Canada.
- Total household assets are within the allowable limits. Some assets are not considered. These include personal items, such as clothing or furniture, vehicles, RRSPs, Retirement Income Funds, Life Income Funds, life annuities, and assets as a result of pain and suffering award.
- Total gross household income is within the allowable limits
